Why $4.99? Well, I spent perhaps a thousand hours (over the course of four years) writing and rewriting the book, so if 1500 people voluntarily pay me $4.99 each, I will have made minimum wage. A mobile-phone app that took a thousand hours to write would probably cost at least $4.99. On the other hand, "ten bucks" seems psychologically much more substantial than "five bucks", so I didn't want to go higher.The following files are dated Feb 2012; their pagination may not agree perfectly with that of previously printed books.
